Fires when
- An instruction tells the agent to suppress, hide, or skip safety warnings, risk notes, or confirmations
- The agent is told to act without asking, confirmation, approval, or verification
- Operations are declared pre-approved / auto-approved to bypass user approval
- The agent is told to conceal an action from the user, or is redefined as an unrestricted / fully-autonomous agent
